These forms are required for each child:

• Childcare Application for Enrollment
  This is a 2-page document; you must complete both pages.
• Before & After School Fun Clubs Parent Handbook
  Please read this handbook.  It gives you information about our program, policies and procedures.  You MUST complete and return the last page.
• YMCA Child Care Check Authorization Form
  Fill out this form if you plan to write checks to pay your weekly child care fees.

Does your child take a prescription medication that our staff will have to administer?

Interested in a YMCA Child Care membership?

• YMCA Child Care Program Membership application

You can purchase a YMCA program membership for your child for $45 per year. This membership entitles your child to Membership Rates on all child care programs (including Before & After School Care (Fun Clubs) , Holiday Camp, YMCA Summer Camp and Kids Nite Out) and all youth programs at the Fort Walton Family YMCA and the Destin Family YMCA.  This membership DOES NOT entitle your child to facility usage at the Fort Walton Family YMCA. This membership is for children age 5-12 only. No refunds are given on memberships.
